We have a very different perspective here in Wales since devolution, when the running of NHS Wales passed to the Welsh Assembly and Executive.
We are six months into a total structure reorganisation with the formation of seven new health boards encompassing primary and secondary care, effectively abolishing the internal market. This gives us the opportunity to completely reorganise and restructure services.
The next Welsh Assembly elections are in 2011 so, at a grassroots level, healthcare delivery will continue largely unchanged until that time.
